My dad spent 40 years as a blue-collar worker; decades of early mornings and long shifts. He was fortunate to work for a company that took safety and health seriously. Because of that commitment, he retired in October 2023 injury and illness-free. Now he’s soaking up retirement with the kind of quality of life every worker hopes for.

That matters to me - not just as his child, but as a safety professional. At Triangle Safety Consulting LLC, we carry that same mindset forward. We believe every worker should make it home safe and healthy every day, all the way to retirement.

We help make that possible through engaging, meaningful safety training and by conducting detailed audits, inspections, and assessments. Because for us, it's personal. It’s not just a box to check. Common Errors I see on OSHA Injury and Illness Recordkeeping and Reporting

For OHS Professionals, maintaining compliance with the Occupational Safety and Health Administration (OSHA) Part 1904 (Recording and Reporting Occupational Injuries and Illnesses) requires an understanding of OSHA 300, 301, and 300A Forms.

Below are the common errors I see when conducting OSHA Compliance audits on OSHA Injury and Illness Recordkeeping and Reporting:

  1. Not having a company executive sign your 300A (STOP signing these if you're a safety professional - unless you are the highest ranking person at your site)
  2. Not adding your recordable injuries to your OSHA 300 log within 7 days of learning of the incident
  3. Improperly identifying if an injury or illness is recordable or reportable (or if it's just first aid)
  4. Not keeping 5 years of OSHA 300, 300A, and 301 Logs
  5. Not adding a Standard Threshold Shift (STS) to the OSHA 300 Log
  6. Not correctly adding up the number of restricted or lost time days
  7. Not reading the standard interpretations for a questionable incident from Part 1904
  8. Not submitting your OSHA 300A Forms to OSHA electronically
